Join Prostate Cancer Voices
You can join Prostate Cancer Voices by completing a registration form. The form can be completed electronically, or printed out and completed by hand. A paper copy of the form can also be posted to you – to request a paper copy please call 0208 222 7622 or email voices@prostate-cancer.org.uk.
How to complete the form
Send the form by post:
- Either complete the form electronically and then print it out
- OR print it out, complete it by hand and post to The Prostate Cancer Charity, FREEPOST, RRLJ-BBSU-SUST, London W6 0LE.
OR, send the form electronically:
- Click on the registration form
- Complete the registration form, which starts on page 5.
- Scroll back up to the top of the document
- Click the ‘Submit by email’ button in the top right hand corner
- Your form will be automatically emailed to us although it will not appear as though anything has happened on your screen
You will receive an email acknowledging receipt of your completed form within 2 working days. I also suggest you save a copy of the form.
